I work for the “Community Connections” Team, part of the County Council, linked to Adult Social Care. We’re a small team of 5 people covering the whole county. We specialise in helping adults with Learning Disabilities and Autistic people to find activities, be they leisure: sport, art, social and volunteering: charity shops, gardening, office admin.
Most of our referrals come from Social Workers, we work closely with Social Care, LD Health and the Employment Service.
We can provide short term 1:1 support and travel training to build the person’s confidence but, we work with people who have the potential to gain more independence. Those with more complex care needs who need ongoing 1:1 support should remain with Adult Social Care.
We have no set time frame for how long we work with someone, and their family but referrals are steady and we have to be sure not to leave people waiting too long.
