There has been a lot of discussion about investigations of serious incidents where individuals have been seriously harmed or have died in NHS care. NHS Improvement is consulting on how serious incidents can be carried out in ways that improve the learning and lead to better outcomes for patients, their families or carers.
This is the link that leads to the consultation document and online questionnaire.The deadline is coming up on 12th June.
It is open to patients, and families who may wish to comment on how systems can be improved to make sure that these investigations lead to real changes in the NHS.
