Healthwatch Oxfordshire want to find out about people’s experiences of obtaining social care assessments and care packages in Oxfordshire.
They would like to understand what the waiting times are like for people to receive their social care assessments as well as their care packages and care reviews.
To this end, they would be very interested to learn more about the experiences of the people you support in getting social care assessments, care packages and care reviews. They have created a short survey online to make it easier for people to provide this feedback and help them with this project.
Please remove any information that contains confidential or personally identifiable details. Your feedback will remain anonymous. If you would like to fill in this form as an individual, rather than a representative of an organisation, please indicate this in the comments box of the survey.
